Before requesting a copy of your sacramental record, please read the information below. Understanding where your records are kept will help ensure that your request is directed to the correct parish.

Your Baptismal Certificate Contains Your Sacramental Record

Your baptismal certificate is the official record that includes information about the sacraments you have received, including Baptism, First Communion, and Confirmation.

If you received your First Communion and/or Confirmation at one of our five family parishes, the parish where you were baptized was notified when you received those sacraments. That information was then added to your baptismal record.

Because of this, you will need to request your baptismal certificate from the church where you were baptized—even if you received your First Communion or Confirmation at a different parish.

Your church of Baptism will have the record of your other sacraments noted on your baptismal certificate.

We understand that this process may seem confusing, but this is the way the Catholic Church maintains official sacramental records.

If you know where you were baptized, please contact that parish to request your baptismal certificate.
